I tried everything, including the things you've written about. No Luck. For years! Sounds like the trouble you've been having. Then I discovered that I was having ground loop problems with my metal rack cabinets. The path was actually from the metal chassis of each unit thru the rack to ALL the other chassis in each rack. I tried a product that isolates each chassis from the rack. Suddenly - no hum! (If I had known that sooner if might have saved my first marriage). Its called Humfrees. Hope this helps.
